Cancellations/Rescheduling of Appointments:

In the low volume type of practice I have developed I am treating one person for 1½ - 2 hours or more at a time. In keeping with the standard practices a minimum of 48-hours of advance notice of your need to cancel or reschedule your appointment is required, with the exception of medical emergencies. In any event I ask that you please give me as much advance notice as possible.

As we establish this professional relationship be assured that I am obligated to adhere to HIPAA regulations. Any & all information that is discussed during your appointments is held in the strictest of confidence, except with your explicit written permission. Additionally, to further protect your privacy & preserve your confidentiality I will never initiate acknowledging you in a public or private venue outside of my office unless you choose to.
